Update to Financials Appendix Form for Security

The Financial Appendix form has been updated to include new eProcurement roles that will be used for Supplier Contract Management for the Health and Human Service Agencies. Some duplicate fields for existing roles have also been removed and the form has been reformatted to keep module roles together. Please use this new version going forward for your agency’s Financial CO-1092 requests.
